Step 3: Fill out the information

  1. First Name
  2. Surname
  3. Email
  4. Select your Organisation Type (e.g. NHS, Primary Care etc)
  5. Select your Organisation
  6. Select your Job Role (see point 7, if yours is not listed) 
  7. Add a customer Job Role
  8. Select the CAPTCHA

Step 4: Press Submit



Step 5: You will receive an email with the following information




Step 6: Log in using your credentials
