Step 3: Fill out the information

- First Name
- Surname
- Email
- Select your Organisation Type (e.g. NHS, Primary Care etc)
- Select your Organisation
- Select your Job Role (see point 7, if yours is not listed)
- Add a customer Job Role
- Select the CAPTCHA

Step 4: Press Submit
